My Doctor, Dr. Kevin Kahn sent the information and request almost two months ago for a authorization for oxymorphone. I have tried to call you over twenty times and told it was a busy time and to try another time. I have never gotten through. I am a chronic pain patient that has been treated for over twenty years by the same doctor. I need to change medicine and you have denied my opportunity to do so by denying the fist medication he suggested and now have not notified myself or my doctor as your decision about this one. I would like to know why humana pharmacy charged me $12. ten more for my order on 10/25/2018 than the past five orders cost of $23. 80 ea. I do believe I have all ready paid my deductible. Thank you in advance,
